Today’s Mantra # 28 ::::: Reason of Problem (RCA) using 5 Why’s
“When we have the courage to ask the 5 Why’s, we gain deeper insight into the issue and identify the true root cause. Once the root cause is known, the chances of finding an effective solution increase greatly. This approach is valuable in both personal and professional life.”
Example: Problem-1: I feel stressed every evening.
Why? Because I have too many things left to do. Why? Because I don’t finish important tasks during the day. Why? Because I get distracted and lose focus. Why? Because I don’t plan my day properly. Why? Because I don’t set priorities in the morning.
✅ Root Cause: No clear daily prioritization
✅ Solution: Start each morning by listing top 3 priorities
Problem-2: A report was submitted late.
Why? Because the data was not ready on time. Why? Because the team received the data input late. Why? Because the requester sent the information after the deadline. Why? Because the requester didn’t know the cut-off timeline. Why? Because the process and expectations were never communicated clearly.

✅ Solution: Share clear deadlines and expectations with stakeholders
